Group Census Form
Needed to get a soft quote (estimated cost).  To get a hard quote (your true cost) we will
need the applications filled out and returned to our office by fax or mail.

Soft quotes take about 3 days.  Hard quotes for 2-50 employees can take 3 to 5 days.  Hard
quotes for 51+ employees can take 7 to 10 days.

*51+ Employees (firm quote) will need (1) 3 years claims experience, (2) How many large
claims over $10,000, (3) Current rates and benefits, (4) 3 years rate and benefit history, (5)
Census or demographics, (6) Plan design and (7) Effective date.


Individuals:
